An LLC filed with the Secretary of State. An NPI number requested. Neither means the agency may serve clients. Many never open.
Licensed
The state health agency issued a license with a date. The agency can now operate, hire, bill, and seek accreditation. That is the list we send.

- State licensing records only: Florida AHCA, Georgia DCH, Arizona ADHS. Every row links to its official record.
- How fresh is it?
- We check weekly. Florida posts licenses 4 to 8 weeks after the license date, so we re-scan a 16-week window and add late rows.
- Do you include emails?
- Georgia publishes agency emails, and we include them as published. Florida does not. In Florida this is a phone-first list.
- Is every row a new business?
- No. Some are ownership changes. We label each row as new agency or ownership change.
- Which states are next?
- New states are added by subscriber demand. Tell us which one you need.
- Can I see it first?
